The factors to be assessed include safety, health and development, cognitive and academic functioning, and social and emotional functioning. WebDCF Permanency planning is the process of assessing and preparing a child for long term care when in out-of-home placements such as kinship, foster care or other placement. A care plan must center on what is in the child’s and families best interest and requires ongoing assessments.
